Saan po ba pwedeng makuha ang MDR?
- Ang MDR ay maaaring makuha sa pinaka malapit na Philhealth Office.
Ano ba ang Members Data Record (MDR)?
- Members Data Records (MDR) ito ay ang dokumento na naglalahad ng lahat ng impormasyon patungkol sa miyembro.
- Dito nakalagay ang buong pangalan ng miyembro, tirahan, istado at kapanganakan nito.
- Nakalagay din dito ang pangalan at lugar na pinapasukan ng miyembro.
- At ang mga ibang taong maaaring makagamit ng benepisyo ng Philhealth tulad ng;
1. anak na may idad na 21 pababa
2. asawa at
3. magulang na may idad na 60 pataas.
- Nakalagay din dito ang eksaktong petsa kung hanggang kailan lamang magagamit ang Philhealth.
